Our doors are always open to new patients.
We see patients of all ages and with various health issues. It is our goal to have you leaving your office visit comfortable and confident with us.
All you need on your first visit is your insurance card and a form of identification (ex. driver’s license, identification card). No need to worry about completing forms prior to your visit, we will collect the rest of the information at our office.

Dr. Zoya Furman, M.D. with AFP Associates is a contracted provider for:
- Most major PPO plans such as Anthem, Aetna, Cigna, Blue Shield and many more.
- Medicare
We do not participate in any HMOs, Medi-Cal or Any Covered California Individual Plans
We collect co-payments at each visit and accept cash, check, or credit cards.
For non-insured patients – we have great affordable options and arrangements available. We will work with you on a payment plan. We are always here to help!
